MDD Congregational Web-site Request Form

Please enter, or attach, informative responses to the prompts listed below. MDD's "webweaver" will then prepare a generic draft "Homepage" for your consideration/modification/approval(rejection). We will mail (USPS or e-mail) a copy for your Board review; and post it to the MDD website for your inspection. Modifications will be accomodated within reasonable limits of practicality. You may preview our recommended Start-up Template, or two sample pages, SAMPLE 1 and SAMPLE 2; presently posted to the MDD website. We also suggest you view other MDD church websites list under "MDD OnLine"

The approved product will then be either,
• posted to an interim website, arranged by MDD, or
• delivered for your disposition by USPS on 3.5" diskette, or attached to an E mail.

We suggest you include (or exclude) the following information:

1.  Your organization name -

2.  Your location; 

	mailing address, 

	directions to your meeting place,

	or other aids to locating you 

3.  Your contact options; 

	phone, 

	FAX, 

	E-mail, 

	semaphore etc. -

4.  Key personalities
	Minister, 

	Officers - 
	      President & others willing to be listed

	Chairs of interest -
	      R.E.;  Alliance;  Key committees  etc.

5.  A paragraph on 
	your philosophy -

	or covenant -

	or similar declaration -

6.  A Logo  &/or  Photo or other unique identifier -

7.  A paragraph about your environment 
	(ie. community, region, state or spiritual entity) -

8.  Your own (optional) suggestions.
